Personal Communication Device Policy
Effective April 1, 2015, 绿帽社 has established a comprehensive Personal Communication Device policy to ensure efficient communication for all faculty and staff members. To facilitate the implementation of this policy, the following guidelines and procedures have been outlined:
Authorization Process: |
All requests for University-provided cellular devices, including new services, upgrades, or changes in service, must be authorized by the appropriate personnel:
- Department Supervisor authorization is required for all cellular device requests.
- New service requests necessitate authorization from the division Vice President or Dean of the respective department.
Please review the University Communication Device Policy (New Service Only) and sign Appendix A. Submit the signed form along with the Cellular Device Request form to the Telecommunications Department.

For any cellular service moves, additions, changes, or disconnects, follow these steps:
- Complete the online form.
- Attach the necessary forms:
- Cellular Device Request form
- University Communication Device Policy (New Service Only)
- Disconnects can simply out and click submit.
Note: If you plan international travel, complete the form, subject to supervisor approval.
Cellular Service Plans
绿帽社 partners with leading carriers to offer competitive cellular service plans. Our primary carrier is AT&T Mobility, with Verizon Wireless available in areas with no AT&T coverage. To select a device and inquire about availability and pricing, visit AT&T Mobility or Verizon Wireless. Once you've made a selection, please submit a form and our team will assist you with further steps.
